Job Description
Implements, coordinates, and evaluates all clinical Pharmacy services, including JCAHO standards. Assists and directs MUEs (Medication Usage Evaluations), collaborates with the medical staff, and establishes criteria for the safe, effective and appropriate use of medication.
1. Develops and monitors the hospital?s policies/procedures regarding utilization of medications in collaboration with the medical staff, Pharmacy, and Therapeutics Committee.
2. Monitors medication utilization effectively and intervenes when necessary to ensure compliance.
3. Monitors pharmacist?s activities regarding utilization surveillance and provides instruction/education as necessary.
4. Maintains necessary and appropriate records regarding the effectiveness of these drug therapy regimens and provides information to QM as required. Provides feedback to the P&T Committee on a regular basis.
5. Evaluates Pharmacy staff regarding safe and effective use of medication.
6. Assists and directs staff in the provision of clinical services.
7. Provides necessary, requested information to management for consideration in the staff?s performance appraisals.
8. Collaboratively, coordinates/implements educational training and competency programs for the staff and pharmacy students.
9. Provides educational programs for health care staff regarding medication use.
10. Provides training as measured by the success of the staff to accomplish identified goals and improve patient care. Incorporates professional concerns into the program.
11. Coordinates activities with the assigned student preceptor and monitors the student?s activities and evaluates performance.
12. Initiates and implements programs with the PCS Division of Education.
13. Coordinates and participates in the development of the hospital?s Formulary and related P&T activities.
14. Contributes to the Formulary review process.
15. Initiates/coordinates MUEs for P&T presentation. Discusses findings at P&T.
16. Performs all duties of a licensed registered Pharmacist as required by the Director and as identified by the ability of the department to effectively provide comprehensive service. Maintains compliance with established procedures, policies, and laws in performing the duties of a licensed, registered staff pharmacist.
17. Performs related duties, as required.
ADA Essential Functions
? Masters Degree in Pharmacy from an accredited college or university, required and two (2) years related experience including medication usage evaluation and distribution in a hospital , required
PharmD and one (1) year PGY residency (or equivalent experience) including medication usage evaluation and distribution in a hospital, required.
Current license to practice Pharmacy in New York State, required.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
